Step 4 Download and fill out the Sample Submission Form (pdf) and print a copy to be submitted with the sample(s).
Step 5 Ship your samples, the Sample Submission Form, a copy of the Purchase Order, and the MSDS to us. If paying by credit card, please include the information on the Sample Submission Form.
*Attention: Please ensure all samples have an SDS and that the SDS is placed in a packing slip envelope on the exterior of the package. Failure to provide an SDS on the Exterior of the package will result in process delays, or the return of your samples without the sample being processed.
Be sure to package your sample(s) carefully for the safety of our staff and the integrity of your data.
The ship to address is on the bottom of the sample submission form. Please do not ship samples using the US Postal Service
Step 6 If this is the first time you have submitted samples to us, we ask that you fill out our NEW CUSTOMER INFORMATION FORM (webpage form). This information will allow us to create your customer account in our system and is only necessary for new customers.
